NEW PATIENT FORMS

New patients and emergencies are always welcome at Binninger Family Dentistry. Typically our relationship begins with a hygiene appointment, where necessary X-rays will be taken and your teeth will be cleaned. We will also review your health and dental history and find out the specifics of your insurance coverage for you. We can explain all your insurance benefits to you and explain how your individual coverage can be best utilized. Dr. Binninger will provide a thorough dental examination and together we will develop an appropriate comprehensive treatment plan that satisfies your needs and addresses any complaints or concerns you may have. 

We like to remind our new patients to bring copies of previous dental records and x-rays to maximize continuity of your care and minimize the duplication of services such as current dental x-rays. If you have dental insurance, we also ask that you bring a copy of your insurance identification card.
